Proper mica insulator use

Sorry for the newbie question...

What is the right way to use a mica insulator on TO-220 parts? It seems
to me that the supplied metal screw and nut would defeat the purpose of
the mica, and conduct the TO-220 case to the heatsink.

Shoulder washer is a plastic insert that seperates the metal screw
physically from the tab on the transistor. The screw is inserted through the
plastic shoulder washer and screwed to the mounting area.

You are missing a little plastic washer/grommet that keeps the metal screw
insulated from the tab.
